Medicare Advantage plans leaving in 2027: what changes in Weber, Davis, Salt Lake, Morgan and Box Elder

The 2027 CMS plan files show fewer Medicare Advantage plans in all five of our counties, with the sharpest drops in Morgan and Box Elder. Here is what changed county by county, how to tell whether your own plan is ending or just changing, and the dates that protect you.

Key takeaways

  • Utah goes from 55 to 51 Medicare Advantage plans statewide for 2027, according to the CMS Utah fact sheet of 28 September 2026.
  • Non-SNP plan counts in the CMS landscape files: Weber 25 → 24, Davis and Salt Lake 31 → 28, Morgan 24 → 16, Box Elder 18 → 12.
  • Box Elder County is down to two Medicare Advantage carriers for 2027 (Humana and UnitedHealthcare), and Morgan County to three (Devoted Health, Humana and UnitedHealthcare).
  • The lowest out-of-pocket maximum in the files rises from $3,500 in 2026 to $4,700 (Weber, Davis, Salt Lake) or $4,900 (Morgan, Box Elder) in 2027.
  • Choose a 2027 plan between 15 October and 7 December for a 1 January start. If your plan is not renewing, a Special Enrollment Period runs 8 December to the end of February.

Every autumn some Medicare Advantage plans change and a few disappear. This year the change is larger than usual in Northern Utah. The 2027 plan files that CMS released in September list fewer Medicare Advantage plans in Weber, Davis, Salt Lake, Morgan and Box Elder counties than the 2026 files did, and two of those counties lose most of their carriers.

If you are enrolled in one of the plans that does not appear in the new file, you should have a letter from your plan by now. If you are in a plan that is staying, you should have an Annual Notice of Change. Either way, the letter in your mailbox is the authority on what happens to you on 1 January, and the rest of this article is about reading it correctly and acting on it in time.

We pulled the 2026 and 2027 CMS landscape files and compared them county by county. The 2026 figures here are the same ones on our county guide pages. What follows is the local picture, the national context, and the dates and rights that apply if your plan is leaving.

Is my Medicare Advantage plan leaving for 2027?

It might be, and the only reliable way to know is the letter your plan sent you. The 2027 CMS landscape files show fewer Medicare Advantage plans in each of the five counties we serve, and several 2026 plans do not appear in the 2027 file at all. A plan missing from the file is either ending or being combined into another plan by the carrier, and only your plan's own notice tells you which.

Statewide, the change is modest. CMS counts 51 Medicare Advantage plans available in Utah for 2027, down from 55 in 2026, and the average monthly Medicare Advantage premium in Utah falls slightly. Every Utah Medicare beneficiary still has access to a Medicare Advantage plan. The statewide number hides how uneven the change is, though, and the uneven part is in our own backyard.

How many Medicare Advantage plans does each county have for 2027?

Fewer in every county, with the largest drops in Morgan and Box Elder. Weber County goes from 25 non-SNP Medicare Advantage plans to 24. Davis and Salt Lake go from 31 to 28 each. Morgan County goes from 24 to 16, and Box Elder County from 18 to 12.

These are net figures. Some plans that do not appear in 2027 were replaced by new ones, so the drop in a county is smaller than the number of plans that went missing from it. Weber is the clearest example: several 2026 plans are absent, but Devoted Health enters the county for the first time and the total falls by only one.

The out-of-pocket maximum is going up

The maximum out-of-pocket (MOOP) is the most you would pay for covered in-network care in a year, and in these files it moves up for 2027. The lowest MOOP offered in any of the five counties was $3,500 in 2026. For 2027 it is $4,700 in Weber, Davis and Salt Lake, and $4,900 in Morgan and Box Elder. The average MOOP rises in every county, by roughly $350 to $530 depending on the county, and the highest MOOP reaches $9,850 everywhere except Box Elder, where it stays at $9,150.

That matters more than the premium for anyone who expects a heavy year. A premium is not the whole cost of a year. The MOOP is the number that decides what a bad year can cost you, so if your plan is staying, check whether its MOOP moved in your Annual Notice of Change.

Which carriers are still offering plans in each county?

Humana and UnitedHealthcare offer non-SNP Medicare Advantage plans in all five counties for 2027; the rest of the list now depends heavily on where you live. HealthSpring has no non-SNP Medicare Advantage plan in any of our five counties in the 2027 file. Devoted Health appears in Weber County for the first time. In Morgan and Box Elder, several carriers that offered plans in 2026 do not appear in the 2027 file at all.

Along the Wasatch Front the carrier list barely moves: one carrier is gone from Davis and Salt Lake, and Weber trades one carrier for another. Morgan and Box Elder are a different story. Morgan goes from seven carriers to three, and Box Elder from five to two. We will come back to what that means for networks, because in a rural county the carrier list is the network list.

Which 2026 plans do not appear in the 2027 file?

The plans below are in the 2026 CMS landscape file for the county shown but have no matching plan ID in the 2027 file. That is all the list means. It does not tell you that any person's coverage is ending, because a carrier can close a plan or fold its members into another plan, and the file alone cannot tell those apart.

In all five counties

  • HealthSpring Premier (HMO-POS) and HealthSpring True Choice (PPO). HealthSpring has no non-SNP Medicare Advantage plan in any of the five counties for 2027.
  • Regence Valiance (PPO).
  • Molina Medicare Complete Care Select (HMO D-SNP), a Special Needs Plan for people with both Medicare and Medicaid.

In Weber, Davis, Salt Lake and Morgan

  • Select Health Medicare + Kroger (HMO).
  • Humana Value Choice (PPO), plan ID H5216-456. A plan named Humana Value Choice (PPO) appears in the 2027 file under a different ID, so this may be a renumbering or crosswalk rather than an exit. Your ANOC will say which plan you are in for 2027.

Also in Morgan County

  • Aetna Medicare Eagle (PPO) and Aetna Medicare Enhanced (PPO)
  • Aetna Medicare Full Dual (HMO D-SNP) and Aetna Medicare Full Dual (PPO D-SNP)
  • Regence MedAdvantage + Rx Classic (PPO) and Regence MedAdvantage + Rx Enhanced (PPO)
  • Select Health Medicare Essential (HMO)

Also in Box Elder County

  • Aetna Medicare Eagle (PPO) and Aetna Medicare Enhanced (PPO)
  • Aetna Medicare Full Dual (PPO D-SNP)
  • Regence MedAdvantage + Rx Classic (PPO)

What is left in Box Elder and Morgan counties for 2027?

Box Elder County has 12 non-SNP Medicare Advantage plans for 2027, all from Humana and UnitedHealthcare. Morgan County has the same 12 plus four more: three from Devoted Health and Humana Value Choice (PPO). Premiums vary by plan.

A dozen plans sounds like plenty of choice until you notice that they come from two companies. In Box Elder County, every non-SNP Medicare Advantage plan for 2027 is built on either a Humana network or a UnitedHealthcare network. If neither network includes the doctors and the hospital you actually use, the number of plans does not help you.

Morgan County has a third option in Devoted Health, which adds plans in the county for 2027. Three networks is better than two, but it is still a short list, and the same check applies: confirm that your doctors, your hospital and your pharmacy are in the network of the specific plan, for 2027, before you enroll.

This is also where a Medicare Supplement deserves a fresh look. A supplement has no network, so the carrier count in your county does not limit it. If your plan is not renewing, you may have a one-time right to buy certain supplements without health questions, which we cover below.

What changed for Special Needs Plans?

Special Needs Plans grow along the Wasatch Front and shrink in the two rural counties. Weber County goes from 13 Special Needs Plans to 17, Davis and Salt Lake from 17 to 18 each, while Morgan goes from 14 to 12 and Box Elder from 12 to 10.

Much of the growth is in chronic-condition plans (C-SNPs). New for 2027 are Devoted Health C-SNP plans and HumanaChoice - Diabetes and Heart (PPO C-SNP). A C-SNP is open only to people with a qualifying condition, such as diabetes or chronic heart failure depending on the plan, and the plan verifies the condition with your doctor.

On the dual-eligible side, Molina Medicare Complete Care Select (HMO D-SNP) does not appear in the 2027 file in any of the five counties, and in Morgan and Box Elder several Aetna Medicare Full Dual D-SNPs are also absent. If you have both Medicare and Medicaid and were in one of those plans, your letter will explain your options, and people with Medicaid have additional enrollment flexibility that is worth asking about.

Is this only happening in Utah?

No. The changes in our counties are part of a national pullback in some Medicare Advantage markets for 2027. Humana said in a 29 July 2026 investor release that it will exit additional Medicare Advantage plans for 2027, affecting roughly 600,000 members nationally. KFF's analysis of 2027 county plan availability found UnitedHealth Group exiting 228 counties and entering 9, and Humana exiting 62 counties and entering 4.

Those are national figures from Humana and KFF, not Utah ones. In our five counties, Humana and UnitedHealthcare are the two carriers still offering non-SNP plans everywhere. We are not going to guess at why any carrier made the choices it did. What matters for you is what is in your county for 2027 and what your own letter says.

How do I tell if my plan is ending or just changing?

Look at which letter you received. If your plan is continuing, even with a new name or a new plan ID, you get an Annual Notice of Change (ANOC), which plans must send so that members receive it by 30 September. If your plan is not renewing, you get a separate non-renewal notice that says the plan is ending and explains your options.

Annual Notice of Change (ANOC)

  • Your plan, or the plan you are being moved into, continues on 1 January
  • Lists changes to premium, copays, MOOP, drug list and network
  • If you are being crosswalked, names the 2027 plan you will be in
  • If you do nothing, you stay enrolled in that plan
  • You can still change plans during Open Enrollment

Non-renewal letter

  • Your plan will not be offered in your area for 2027
  • Explains that your coverage in the plan ends
  • Explains your right to choose another plan
  • Opens a Special Enrollment Period
  • Can give you a Medigap guaranteed-issue right
  1. Find the letter. It came from your plan, not from Medicare. The ANOC should have reached you by 30 September. Look for the words "Annual Notice of Change" or "non-renewal" on the first page.
  2. Check the plan name and ID for 2027. If the ANOC names a different plan from the one on your card, you are being moved, and you should read the new plan's benefits as if you were choosing it fresh.
  3. Check your doctors and drugs against the 2027 plan, not the 2026 one. Networks and drug lists change each year.
  4. If you cannot find a letter, call the plan. The member services number is on your card. Ask directly: "Is my plan renewing for 2027, and if not, what plan am I being moved to?"

When should I choose a new plan if mine is leaving?

Ideally during Open Enrollment, between 15 October and 7 December 2026, so your new plan starts on 1 January 2027 with no gap. That is the cleanest path. If you miss it, a Special Enrollment Period for people whose plan is not renewing runs from 8 December 2026 to the last day of February 2027.

The Special Enrollment Period is a safety net, not a plan. If you use it in January, your new plan starts on 1 February, which means January is spent in Original Medicare without the plan you will eventually choose. Deciding before 7 December avoids that month altogether.

Can I switch to a Medicare Supplement without health questions?

Yes, if your Medicare Advantage plan is leaving Medicare or stops providing care in your area, you have a federal guaranteed-issue right to buy certain Medicare Supplement (Medigap) plans without medical underwriting. You can apply as early as 60 days before your Medicare Advantage coverage ends and no later than 63 days after it ends.

  • Plans you can buy under this right: Medigap Plan A, B, K or L sold in Utah by any insurance company.
  • If you were eligible for Medicare before 1 January 2020: you can also buy Plan C or Plan F.
  • If you became eligible on or after 1 January 2020: you can buy Plan D or Plan G in place of C and F.
  • The right applies only if you move to Original Medicare. It does not apply if you join another Medicare Advantage plan.
  • Medigap cannot start until your Medicare Advantage coverage ends. Apply early, and set the start date to the day after your plan ends.

This matters in Utah because outside your first six months on Part B, Utah carriers can generally ask health questions and decline you for a Medigap plan. A non-renewal can be a rare second door. Plan G, for example, is available under this right to people who became eligible for Medicare on or after 1 January 2020. If a supplement has ever appealed to you, this is the moment to price it.

A supplement covers medical care only. You would also need a stand-alone Part D plan for prescriptions, and the premium for both is real money every month. Our Medigap Plan G in Utah guide covers how to compare it.

What happens if I do nothing?

If your plan is not renewing and you do nothing, you go back to Original Medicare when the plan ends. You keep Medicare Part A and Part B, but you lose the plan's other benefits and, if your plan included drug coverage, your prescription coverage too.

That last part is the risk. Original Medicare does not cover most outpatient prescriptions, so without a Part D plan you pay full price at the pharmacy. You can join a stand-alone Part D plan using the same non-renewal Special Enrollment Period, but every month without drug coverage is a month of full-price prescriptions.

If your plan is not leaving and you do nothing, you stay in it for 2027 under the new terms in your ANOC. That can be fine, but it is still worth reading the letter, because premiums, copays, the MOOP and the drug list can all change.

What should I do this week?

Find your letter, confirm what it says, and make a decision before 7 December. Open Enrollment opened today, 15 October, and the earlier you start, the more time you have to check doctors and drugs properly.

  1. Find your ANOC or non-renewal letter and read the first page.
  2. Write down your doctors, your hospital, your pharmacy and every prescription, with strength and dose.
  3. List the 2027 plans in your county. In Box Elder that means Humana and UnitedHealthcare plans; in Morgan, those plus Devoted Health.
  4. Check each plan's network and drug list for 2027, not 2026.
  5. Compare the MOOP as well as the premium, since the lowest MOOP in our counties rises to $4,700 or $4,900 for 2027.
  6. If your plan is not renewing, price a Medicare Supplement plus Part D while your guaranteed-issue right is open.
  7. Enroll by 7 December for a 1 January start, and keep the confirmation.

The bottom line

The 2027 CMS files show fewer Medicare Advantage plans in all five of our counties, a higher floor on out-of-pocket maximums, and a much shorter carrier list in Morgan and Box Elder. None of that tells you what is happening to your own plan, because your letter does that. Read it, check your doctors and drugs against the 2027 plans in your county, and decide before 7 December so your new coverage starts on 1 January with no gap. If your plan is not renewing, remember the Special Enrollment Period and your Medigap guaranteed-issue right. Grab a Cup. Let's Get You Covered.

Frequently asked questions

How do I know if my Medicare Advantage plan is ending in 2027?

Your plan's own letter tells you. A plan that is continuing sends an Annual Notice of Change, which members should receive by 30 September; a plan that is not renewing sends a separate non-renewal notice. If you have neither, call the member services number on your plan card and ask whether your plan is renewing for 2027.

My plan is not in the 2027 CMS file for my county. Does that mean it is cancelled?

Not necessarily. A plan that does not appear in the 2027 CMS landscape file for your county is either not renewing or being combined into another plan by the carrier, which is called a crosswalk. Your ANOC or non-renewal letter says which. Humana Value Choice (PPO) is one example: the 2026 plan ID is missing, but a plan with the same name appears in 2027 under a new ID.

How many Medicare Advantage plans are there in my county for 2027?

In the 2027 CMS landscape file, not counting Special Needs Plans: Weber County has 24 (down from 25), Davis and Salt Lake have 28 each (down from 31), Morgan has 16 (down from 24) and Box Elder has 12 (down from 18). Statewide, CMS counts 51 Medicare Advantage plans in Utah for 2027, down from 55.

Which Medicare Advantage carriers are in Box Elder County for 2027?

Two, for non-SNP plans: Humana and UnitedHealthcare. Aetna Medicare, HealthSpring and Regence BlueCross BlueShield of Utah offered plans in Box Elder in 2026 but do not appear in the 2027 file for the county. Morgan County has three carriers for 2027: Devoted Health, Humana and UnitedHealthcare.

When can I change plans if mine is not renewing?

During Open Enrollment, 15 October to 7 December 2026, for a 1 January 2027 start. If you miss that, a Special Enrollment Period for non-renewals runs from 8 December 2026 to the end of February 2027. Requests made 8 to 31 December take effect 1 January; requests in January take effect 1 February.

What happens if my plan is ending and I do nothing?

You return to Original Medicare when the plan ends. You keep Part A and Part B, but you lose the plan's other benefits and any drug coverage it included. You would need to join a stand-alone Part D plan for prescriptions, and going 63 days or more in a row without creditable drug coverage can lead to a permanent late enrollment penalty.

Can I buy a Medicare Supplement without health questions if my plan leaves?

Yes, if your Medicare Advantage plan is leaving Medicare or stops providing care in your area and you move to Original Medicare. You can buy Medigap Plan A, B, K or L; Plan C or F if you were eligible for Medicare before 1 January 2020; or Plan D or G if you became eligible on or after that date. Apply from 60 days before your coverage ends to 63 days after it ends.

Is the out-of-pocket maximum going up in 2027?

In the CMS files for our five counties, yes. The lowest MOOP offered was $3,500 in 2026; for 2027 it is $4,700 in Weber, Davis and Salt Lake and $4,900 in Morgan and Box Elder. The average MOOP rises in every county. If your plan is staying, check its 2027 MOOP in your ANOC.

Is Devoted Health new in Weber County?

Yes. Devoted Health appears in Weber County for the first time in the 2027 CMS landscape file, with three non-SNP PPO plans plus chronic-condition (C-SNP) plans and a dual-eligible (D-SNP) plan. Devoted also adds plans in Davis, Salt Lake and Morgan counties for 2027.

Is this happening outside Utah too?

Yes. Humana said in a 29 July 2026 investor release that its 2027 Medicare Advantage plan exits affect roughly 600,000 members nationally, and KFF's analysis of 2027 county plan availability found UnitedHealth Group exiting 228 counties and Humana exiting 62. Those are national figures; your own county list and your own letter are what matter for you.
